Research & Scholarship

Current Research and Scholarly Interests

At Stanford Coordinated Care we are developing and researching a model of care for patients with complex chronic medical conditions who are not 'doing well'. We will assess four domains for each patient: the medical neighborhood; social support; self-management; and medical status and help patients develop their skills and support systems in these areas. Our goal is the triple aim: improved patient experience, improved medical outcomes and cost savings.
